Club Pooch Complete Canine Facility
Release Agreement & Boarding/Daycare Contract

  1. I hereby certify that I am the owner and/or responsible party for the pet(s) listed below and wish to have the below listed pet(s) under the care and protection of Club Pooch and their employees.

  2. All pets are to be currently vaccinated. Should club personnel of Club Pooch be bitten or otherwise exposed to any disease or ailment received from Client’s animal(s), it will be the Client’s responsibility to pay all costs and damages incurred. Kennel Cough shot is required and clean fecal exam is required from veterinarian every six (6) months.

  3. I understand that the services/activities offered by Club Pooch may involve the interaction of a variety of breeds of dogs with different temperaments and that my pet(s) may have interaction with these animals at any given time.

  4. I agree that in allowing me to bring my pet(s) to Club Pooch to partake in the services/activities provided, the owners and employees of Club Pooch have relied upon the information I have provided to them regarding the health, temperament, and training of my pet(s) and that Club Pooch, its owners, operators, employees and/or agents are not responsible for deciding whether my pet(s) should participate in these activities. I further certify that I have provided Club Pooch, its owners, operators, employees and/or agents with all the information they have requested and all other information in my possession or control related to the health, temperament, training or other characteristics of my pet(s).

  5. I further agree that Club Pooch, its owners, operators, employees and/or agents are not liable to me or any other person for any problems, incidents, or injuries that result from the services/activities offered by Club Pooch, its owners, operators, employees and/or agents. I understand that at times Club Pooch facility may offer additional dog related activities that may be potentially, inherently dangerous to pet(s) and people who participate, and that I am assuming all risk of injury to myself or my pet(s).

  6. I additionally agree to hold harmless Club Pooch, it owners, operators, employees, and/or agents for any claims made by any individual, corporation or other entity for any injury or other harm sustained by that entity as a result of the actions of my pet(s). I understand that I am solely responsible for any such claims, regardless of the nature of the events that underlie such a claim and that Club Pooch, its owners, operators, employees, and/or agents are not responsible for the actions of my pet(s) while they participate in the offered services/activities.

  7. I understand that Club Pooch, its owners, operators, employees, and/or agents will use their best judgment in the operation of the offered activities, and I agree that they may act in their sole discretion in that judgment and I further agree to assume full financial and legal responsibility for any and all expenses or liabilities that may result from any actions necessary to implement their judgment. Also in the event of inclement weather or natural disaster, Club Pooch is entrusted to use best judgment in caring for pet(s). Club Pooch will be held harmless for consequences to such decisions.

  8. I understand that these rules and regulations may be altered or amended at any given time. I agree that, upon receipt of a copy of said changes, I will be deemed to have agreed to all such changes by the continued participation of my pet(s) in the offered services/activities.

  9. BOARDING CANCELLATION/RESERVATION POLICY: Space is limited and reservations are required. Reservations should be made as far in advance as possible. If notice of cancellation is given at least two weeks in advance of start date, 100% of fees will be credited to client’s Club Pooch account. If notice of cancellation is given one week prior to start day, 50% of fees will be credited to client’s Club Pooch account. If notice of cancellation is given with less than one full week prior notice, no credit will be given.

  10. DAYCARE CANCELLATION/RESERVATION POLICY: Space is limited and reservations are required. Reservations should be made as far in advance as possible.No refund on Daycare Packages.

I HEREBY CERTIFY THAT I HAVE READ AND UNDERSTAND ALL OF THE ABOVE ITEMS, AS WELL AS, ANY OTHER RULES AND REGULATIONS THAT HAVE BEEN PROVIDED TO ME BY CLUB POOCHE. I AGREE TO ABIDE BY THESE RULES AND REGULATIONS AND I ACCEPT THE TERMS, CONDITIONS AND STATEMENTS SET OUT HEREWITH.
